About the Role

We are seeking a highly motivated and experienced Support Worker to join our Higher Needs Floating Support Service team. As a Support Worker, you will work closely with clients who are facing multiple disadvantages and are at risk of homelessness. Your primary goal will be to empower clients to take control of their lives and make informed decisions about their support needs.

Key Responsibilities

• Work with clients to identify and address their complex needs and develop a comprehensive support plan to achieve their goals.

• Collaborate with local authorities, healthcare providers, and other stakeholders to ensure a coordinated approach to supporting clients.

• Develop and maintain a network of support for clients, including landlords, support agencies, and community resources.

Requirements

• Proven experience of working with clients who have complex and multiple needs, including mental health, drug and alcohol, homelessness, and offending.

• Experience of outreach or floating support work, with a proven track record of engaging clients and developing effective support plans.

• Ability to work collaboratively with clients, colleagues, and external agencies to achieve shared goals.

• Strong communication and interpersonal skills, with the ability to build relationships with clients and stakeholders.
