About the RoleThis is a challenging and rewarding opportunity for a registered nurse with at least 6 months of dialysis experience. As a Clinical Nurse Coordinator, you will be responsible for coordinating patient care and services, including tracking lab values, vital signs, and outcome management. You will work closely with our interdisciplinary team to ensure the safety, comfort, and well-being of our patients.
Responsibilities- Coordinate patient care and services, including tracking lab values, vital signs, and outcome management.
- Ensure the highest level of care for patients with end-stage renal disease or chronic kidney disease.
- Work closely with the interdisciplinary team to ensure patient safety, comfort, and well-being.
- Participate in quality improvement initiatives to improve patient outcomes.
- Current Registered Nurse (RN) license in the state of practice.
- At least 18 months of registered nursing experience.
- Dialysis Charge RN readiness approval by Clinical Nurse Manager or RN-licensed FA/GFA required.
- Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN) preferred.
- Intensive Care Unit (ICU), Critical Care Unit (CCU), Emergency Room (ER), or Medical Surgical (Med Surg) experience is preferred.
