Mental Health Social Worker Opportunity

We are seeking a dedicated and compassionate Mental Health Social Worker to join our team at Tripod Partners. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality assessments, interventions, and support to individuals experiencing mental health challenges.

About the Role:

As a Mental Health Social Worker, you will work collaboratively with service users, their families, and other healthcare professionals to promote recovery, independence, and well-being. Your key responsibilities will include:

  • Conducting thorough assessments of individuals' mental health needs and risks
  • Developing and implementing person-centred care plans in collaboration with service users and their families
  • Providing ongoing support and intervention to help individuals manage their mental health and achieve their goals
  • Working closely with other health and social care professionals to deliver integrated care
  • Engaging in safeguarding processes to protect vulnerable individuals and ensure their safety
  • Maintaining accurate case records and documentation in line with relevant policies and procedures
