Job Summary

The Bramhall and Cheadle Hulme Primary Care Network is seeking an experienced Advanced Nurse Practitioner to join our team. As an Advanced Clinical Practitioner, you will provide high-quality patient care, working within your professional boundaries to assess, diagnose, and treat patients. You will be responsible for managing undifferentiated conditions, identifying red flags, and taking appropriate action.

Main Responsibilities

  • Assess and triage patients, providing definitive treatment or making necessary referrals.
  • Manage undifferentiated conditions, identifying red flags and underlying serious pathology.
  • Use complex decision-making to inform diagnosis, investigation, and management of episodes of care.
  • Take a personalised care approach, enabling shared decision-making with patients.
  • Provide direct access to a nurse practitioner for the general practice population.
  • Make home visits to patients who are too unwell to attend the clinic.
  • Make professionally autonomous decisions, taking responsibility for your actions.
  • Provide assistance to GPs, referring patients to other services as necessary.
  • Instigate diagnostic tests and investigations, interpreting findings and reports.
  • Prescribe safe and effective medication, adhering to the Practice prescribing formulary.
  • Provide safe, evidence-based, and cost-effective patient care.
  • Offer a holistic service, developing ongoing plans of care and support.
  • Promote health and well-being through health promotion, education, and communication skills.

Person Specification

  • Evidence of Continued Professional Development.
  • Experience of working in Primary Care.
  • Ability to prioritise and organise your case load.
  • Excellent communication and interpersonal skills.
  • Ability to evaluate and implement clinical service developments.
  • Ability to use in-house Patient Databases.
  • Experience of working within a team.
  • Willingness to undertake further training as required.

Qualifications

  • Masters qualification for Advanced Clinical Practice or similar accreditation.
  • Independent Prescribing qualification.
  • NMC registration.
