Job Summary

We are seeking an experienced Advanced Clinical Practitioner to join our Urgent Community Response and Virtual Ward Team at Stockport Foundation Trust. The successful candidate will work autonomously, managing a caseload of patients and clinical priorities according to agreed protocols and working practices.

The ideal candidate will have a strong background in advanced clinical practice, with experience in community or a relevant specialty area. They will be able to demonstrate excellent communication skills, problem-solving abilities, and the ability to work independently and as part of a team.

The Trust is committed to providing high-quality patient care, and the successful candidate will be expected to contribute to this goal through their work. They will also be expected to participate in education and research activities, and to contribute to the development of the Trust's policies and procedures.

Key Responsibilities

  • Work autonomously as an ACP within the specialty, managing a caseload of patients and clinical priorities according to agreed protocols and working practices.
  • Utilise scope of advanced practice to undertake Non-Medical Prescribing role and advise on medicine management issues associated with the patient group, working within Trust policy for Medicines Management.
  • Contribute to the development of the Trust's policies and procedures, and participate in education and research activities.

Person Specification

  • Professional Registration
  • Non-Medical prescribing course (in eligible professions)
  • MSc in clinical practice
