Job Title: Advanced Nurse Practitioner

Job Summary:

The Advanced Nurse Practitioner will provide high-quality patient care, working autonomously and as part of the GP clinical team across practices within the Primary Care Network. The successful candidate will have a Master's qualification for Advanced Clinical Practice and independent prescribing qualification, with experience of working in primary care and a strong ability to evaluate and implement clinical service developments.

Main Responsibilities:

  • Assess and triage patients, providing definitive treatment or making necessary referrals to other members of the primary care team.
  • Manage undifferentiated undiagnosed conditions, identifying red flags and underlying serious pathology, and take appropriate action.
  • Use complex decision-making to inform diagnosis, investigation, and complete management of episodes of care within a broad scope of practice.
  • Take a personalised care approach and population-centred care approach to enable shared decision-making with patients.
  • Provide direct access to a nurse practitioner for the general practice population, making home visits as necessary.
  • Make professionally autonomous decisions for which you are accountable.
  • Provide assistance to GPs seeing and treating patients referred to you as appropriate.
  • Instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports.
  • Prescribe safe, effective, and appropriate medication as defined by the current legislative framework.
  • Provide safe, evidence-based, cost-effective, individualised patient care.
  • Offer a holistic service to patients and their families, developing an ongoing plan of care/support.
  • Promote health and well-being through health promotion, health education, screening, and therapeutic communication skills.
  • Refer patients directly to other services/agencies as appropriate.
  • Work with the team within the Practice, and across the wider NHS, to promote integrated and seamless pathways of care.
  • Contribute to the practice achieving its quality targets to sustain high standards of patient care and service delivery.
  • Participate in the identification of community health needs and develop patient/family-centred strategies to address them.

Person Specification:

  • Evidence of Continued Professional Development.
  • Experience of working in Primary Care.
  • Ability to prioritise and organise own case load.
  • Excellent communication and interpersonal skills.
  • Ability to evaluate and implement appropriate clinical service developments.
  • Ability to use in-house Patient Databases.
  • Experience of working within a team.
  • Willingness to undertake further training as required.

Qualifications:

  • Masters qualification for Advanced Clinical Practice or similar accreditation.
  • Independent Prescribing qualification.
  • NMC registration.
