Job Summary

The Bramhall and Cheadle Hulme Primary Care Network is seeking an experienced Advanced Nurse Practitioner to join our team. As an Advanced Nurse Practitioner, you will provide high-quality patient care, working autonomously and as part of the GP clinical team across practices within the network.

Main Responsibilities

  • Assess and triage patients, providing definitive treatment or making necessary referrals to other members of the primary care team.
  • Manage undifferentiated undiagnosed conditions, identifying red flags and underlying serious pathology, and taking appropriate action.
  • Use complex decision-making to inform the diagnosis, investigation, and complete management of episodes of care within a broad scope of practice.
  • Take a personalised care approach and population-centred care approach to enable shared decision-making with patients.
  • Provide direct access to a nurse practitioner for the general practice population, making home visits as needed, and prescribing safe and effective medication.
  • Instigate necessary invasive and non-invasive diagnostic tests or investigations, interpreting findings and reports.
  • Work with the team to promote integrated and seamless pathways of care, contributing to the practice achieving its quality targets.
  • Participate in the identification of community health needs and develop patient/family-centred strategies to address them.

Person Specification

  • Evidence of Continued Professional Development.
  • Experience of working in Primary Care.
  • Ability to prioritise and organise own case load.
  • Excellent communication and interpersonal skills.
  • Ability to evaluate and implement appropriate clinical service developments.
  • Ability to use in-house Patient Databases.
  • Experience of working within a team.
  • Willingness to undertake further training as required.
  • Masters qualification for Advanced Clinical Practice or similar accreditation.
  • Independent Prescribing qualification.
  • NMC registration.
