Overview

Bramhall and Cheadle Hulme Primary Care Network is seeking an experienced Advanced Nurse Practitioner to join our team. This is a fantastic opportunity to work in a dynamic and supportive environment, delivering high-quality patient care.

About the Role

The successful candidate will be responsible for providing expert clinical care to patients within our primary care network. This includes assessing and triaging patients, managing undifferentiated conditions, and using complex decision-making to inform diagnosis and treatment.

Main Responsibilities

  1. Assess and triage patients, including same-day triage, and provide definitive treatment or make necessary referrals to other members of the primary care team.
  2. Manage undifferentiated undiagnosed conditions and identify red flags and underlying serious pathology, taking appropriate action.
  3. Use complex decision-making to inform the diagnosis, investigation, and complete management of episodes of care within a broad scope of practice.
  4. Actively take a personalised care approach and population-centred care approach to enable shared decision-making with the presenting person.
  5. Complete relevant training to provide multi-professional clinical practice and CPD supervision to other roles within primary care.
  6. Provide direct access to a nurse practitioner for the general practice population and make home visits to those who are too unwell to come into the clinic in person.
  7. Make professionally autonomous decisions for which you are accountable and provide assistance to GPs seeing and treating patients referred to you as appropriate.
  8. Instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports.
  9. Prescribe safe, effective, and appropriate medication as defined by the current legislative framework in accordance with the Practice prescribing formulary (or generically) whenever this is clinically appropriate.
  10. Provide safe, evidence-based, cost-effective, individualised patient care and offer a holistic service to patients and their families, developing where appropriate an ongoing plan of care/support.
  11. Promote health and well-being through the use of health promotion, health education, screening, and therapeutic communication skills.
  12. Refer patients directly to other services/agencies as appropriate and work with the team within the Practice, and across the wider NHS, to promote integrated and seamless pathways of care.
  13. Contribute to the practice achieving its quality targets to sustain the high standards of patient care and service delivery.

Requirements

To be successful in this role, you will need:

  • Evidence of Continued Professional Development.
  • Experience of working in Primary Care.
  • Ability to prioritise and organise own case load.
  • Excellent communication and interpersonal skills.
  • Ability to evaluate and implement appropriate clinical service developments.
  • Ability to use in-house Patient Databases.
  • Experience of working within a team.
  • Willingness to undertake further training as required.
  • Masters qualification for Advanced Clinical Practice or similar accreditation.
  • Independent Prescribing qualification.
  • NMC registration.

Salary Information

The salary for this position is estimated to be around £55,000 - £65,000 per annum, depending on experience.
