Nurse Practitioner

We are seeking a skilled and experienced Nurse Practitioner to join our District Nursing Team. As a Triage Nurse, you will be responsible for assessing patients' needs and prioritizing treatment to ensure the right care is delivered at the right time.
Key Responsibilities:
- Assess patients' needs and prioritize treatment
- Make and receive referrals from General Practitioners and other health professionals
- Work with the Community Nurses and Adults Social Care staff to provide a first-class service to the residents of Stockport
Requirements:
- District nursing experience and knowledge
- Excellent clinical skills and IT skills
- Ability to work flexibly and as part of a team
