About the Role

We are looking for a Heart Failure Nursing Expert to join our team at Oxford University Hospitals NHS Foundation Trust. As an expert in heart failure nursing, you will be responsible for providing high-quality care to patients with chronic heart failure.

Key Responsibilities:

  • Assess and develop individualized care plans for patients with chronic heart failure.
  • Collaborate with multidisciplinary teams to ensure comprehensive care.
  • Educate patients and their families on self-care techniques and disease management.
  • Contribute to quality improvement initiatives and clinical research projects.
  • Develop and implement strategies to improve patient outcomes and reduce readmissions.

Requirements:

  • Registered nurse with experience in heart failure nursing or cardiovascular rehabilitation.
  • Strong communication and interpersonal skills.
  • Ability to work effectively in a team environment.
  • Knowledge of evidence-based practice guidelines for heart failure care.

Salary: £45,000 - £65,000 per annum, depending on experience.
