The Advanced Heart Care Coordinator plays a crucial role in enhancing the quality of life for patients with chronic heart failure at the Oxford University Hospitals NHS Foundation Trust. This is an exciting opportunity for an experienced Registered Nurse to contribute to the delivery of high-quality, evidence-based care and make a meaningful impact on patient outcomes.

About the Position

The successful applicant will work in partnership with other healthcare professionals to develop and implement personalized care plans, promote healthy behaviors, and educate patients and carers about self-management techniques. The ideal candidate will possess excellent organizational and communication skills, a strong commitment to continuous learning, and experience working in multidisciplinary teams.

Duties and Responsibilities
  • Collaborate with the multiprofessional team to develop a shared vision of the service, contributing to the development of effective documentation, pathways, protocols, and guidelines for care.
  • Contribute to the evaluation of the service and patient care, including monitoring of patient experience, and identify areas of risk and poor quality to address through governance structures and forums.
Compensation and Benefits

We offer a salary range of £42,000 - £52,000 per annum, depending on experience, plus benefits including pension scheme, annual leave, and training opportunities.
