Nurse Practitioner in Community Healthcare

We are seeking a highly skilled and experienced Nurse Practitioner in Community Healthcare to join our team in Lincolnshire. As a key member of our team, you will work closely with children, families, and other healthcare professionals to provide high-quality care and support in a community setting.
Your main duties will include managing a caseload of children with complex needs, coordinating services to facilitate early discharge, and participating in an evening telephone on-call system to support all children and families on the CCN countywide caseload.
As an excellent communicator, you will also be responsible for developing ambulatory care pathways and promoting the ethos of empowering children and their families to develop the knowledge, skills, and expertise to enable them to become self-caring, independent, and manage their own condition at home wherever possible.
We offer a competitive salary of £37,338 to £44,962 per year (pro rata for Part-time) and opportunities for further professional development, training, and regular 1:1 support from the Team Leader.
