An exciting opportunity has arisen for experienced and highly motivated individuals to join Homerton Healthcare NHS Foundation Trust as a Community Nursing Team Leader.

As a Clinical Lead for Community Care, you will be responsible for ensuring the operational continuity, financial management and clinical effectiveness of care delivery in designated Neighbourhoods. This includes leading a team of Specialist Practitioner District Nurses, community nurses and health care support workers in delivering high standards of care to patients.

Main Responsibilities

You will work within the Neighbourhoods Community Nursing Team for Homerton University Hospital and collaborate with other professional services, including acute hospitals, providing leadership to the community nursing team.

Person Specification
  • NMC Registration and SPDN Qualification
  • Extensive experience in community nursing, ideally in a leadership capacity
  • Strong clinical leadership skills and ability to act as a clinical leader
  • Excellent communication and interpersonal skills, with the ability to work collaboratively within a multi-disciplinary team

The estimated salary for this role is £35,000 - £45,000 per annum, depending on experience.
