Community Nursing Team Leader

We are seeking a highly skilled and experienced Registered Nurse to lead our Community Nursing Team. This is an exciting opportunity to work in a dynamic and supportive environment, providing high-quality care to patients in their homes.
Key Responsibilities- Lead and manage a team of community nurses, providing clinical guidance and supervision as needed.
- Develop and implement effective care plans, working closely with patients, families, and other healthcare professionals.
- Assess and manage patient needs, prioritizing care and ensuring seamless transitions between services.
- District nurse qualification and current NMC registration.
- Experience in team leadership, teaching, and assessing individuals.
- Excellent communication and interpersonal skills.
Salary: £45,751 - £60,461 per annum (dependent on experience)
