Job Overview

We are seeking a skilled and experienced District Nurse Team Leader to join our Brent Community Nursing Team at CENTRAL LONDON COMMUNITY HEALTHCARE NHS TRUST.

Role Summary

The successful candidate will be responsible for leading a team of nurses and healthcare assistants, providing high-quality patient care in a community setting. The role involves managing a caseload, delivering evidence-based nursing care, and coordinating discharge planning to prevent hospital admissions.

Key Responsibilities
  • Lead a team of nurses and healthcare assistants to provide high-quality patient care.
  • Manage a caseload and deliver evidence-based nursing care.
  • Coordinate discharge planning to prevent hospital admissions.
  • Work closely with GPs, social care, and other health and voluntary services to provide seamless, integrated care.
Requirements
  • Registered General Nurse with current UK NMC registration.
  • Specialist Practitioner qualification in District Nursing or working towards it.
  • Proven experience in community nursing with a focus on complex care, team coordination, and multidisciplinary collaboration.
  • Strong leadership skills with the ability to motivate, delegate, and handle complex care decisions.
Salary and Benefits

The estimated salary for this role is £62,000 - £83,000 per annum, depending on experience. The role also offers flexible working options, car lease schemes, annual travel card loans, and continuous training and development opportunities.
