About the Role
This role is an exciting opportunity to join Midlands Partnership NHS Foundation Trust as a Health Needs Assessment Practitioner. You will be working in Liaison and Diversion services, identifying individuals with substance misuse, mental health, learning disability, or other vulnerabilities within the criminal justice system.

Key Responsibilities

  • Conduct holistic health assessments in Liaison and Diversion, collaborating with service users and carers when necessary, to promote self-determination, choice, and autonomy.
  • Complete and contribute to robust risk assessments and risk management plans, delivering positive risk taking where appropriate to enhance quality of life and maximize occupational performance/independence.
  • Maintain an effective reporting system by observing and reporting verbally and electronically in health records.
  • Provide feedback to youth justice panels and Youth Offending services.
