About the Role

We are seeking a highly motivated and organized Hospital Liaison Specialist to join our team in Stoke on Trent. As a key member of our recovery care team, you will work closely with local hospitals to provide high-quality support to individuals struggling with substance misuse.

Main Responsibilities
  • Establish and maintain effective relationships with hospital staff and clients
  • Provide ongoing support and guidance to clients in the community
  • Collaborate with other agencies to ensure a comprehensive service
  • Conduct assessments and risk assessments to identify client needs
  • Develop and implement personalized support plans
Requirements
  • ONC/NVQ level 3 or equivalent
  • Experience working in a health or social care setting
  • Knowledge of health and safety protocols
  • Ability to work effectively in a team
  • Excellent communication and interpersonal skills
