Hospital Link Worker

We are seeking a highly motivated and organized individual to join our team as a Hospital Link Worker. As a key member of our team, you will work closely with our clients and local hospitals to establish and maintain a high-quality hospital liaison service for those suffering from substance misuse health needs.
Key Responsibilities:
- Work with clients and local hospitals to establish and maintain a high-quality hospital liaison service
- Provide support and guidance to clients in the community
- Collaborate with other agencies to ensure a comprehensive service is provided
- Develop and maintain effective relationships with clients and colleagues
What We Offer

Person Specification
To be successful in this role, you will need to have:
- Attained ONC/NVQ level 3 or be willing to achieve this within 12 months
- Experience of working in a health, social care, or related setting
- Knowledge and understanding of health and safety, including assessing high-risk situations for clients
- A successful track record of working with those who struggle with substance misuse
