Job Description

East Lancashire Alliance is seeking a highly skilled Children and Families Practitioner to join our team. As a key member of our healthcare team, you will be responsible for providing safe, evidence-based, and cost-effective patient care to children and families within our surgery, their own homes, or other environments where care is delivered.

Key Responsibilities:

  • Assess and Plan Care: Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly, referring patients to other services or agencies as needed.
  • Investigatory Procedures: Recognise the need for and be able to perform and interpret investigatory procedures, including laboratory and radiology.
  • Emergency Situations: Prioritise health problems and intervene appropriately to assist patients in complex, urgent, or emergency situations, including the initiation of effective emergency care.
  • Health Promotion: Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care.
  • Professional Conduct: Recognise and work within your own competence and in accordance with professional codes of conduct, maintaining accurate and contemporaneous health records.
  • Prescribing and Medication: Compile and issue computer-generated acute and repeat prescriptions, and review medications following appropriate policies, NICE/local clinical guidelines, and local care pathways.
  • Confidentiality and Health and Safety: Maintain confidentiality and adhere to our policies and procedures on safeguarding children and adults, as well as health and safety protocols.
  • Quality and Risk Management: Contribute to the achievement of the highest possible quality standards, monitor the safety and effectiveness of own clinical practice, and participate in Significant Event and/or near miss analysis reviews.

Requirements:

  • Professional Qualifications: Relevant professional qualifications and registration with the relevant governing body.
  • Experience and Skills: Proven experience and skills in providing patient care, assessing patient needs, and working within a multidisciplinary team.
  • Communication and Interpersonal Skills: Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Continuous Professional Development: Commitment to ongoing learning and professional development, with the ability to adapt to changing healthcare policies and guidelines.
