Job Title: Community Mentor

Estimated Salary: $17 per hour

Job Description:
  1. Deliver person-centered care and support to individuals in their homes and communities.
  2. Teach life and social skills to empower individuals to live independently.
  3. Collaborate with families to develop and implement personalized plans.
  4. Ensure individuals receive comprehensive support for their physical, emotional, and social well-being.
Responsibilities:
  • Assist with activities of daily living, such as personal care, meal preparation, and household chores.
  • Teach and promote healthy habits, including nutrition, exercise, and safety practices.
  • Facilitate connections between individuals, families, and community resources.
  • Monitor and report on individual progress, providing recommendations for improvement.
Qualifications:
  • High School diploma required; BA degree in Human Services or related field preferred.
  • At least 1 year of experience supporting adults with developmental disabilities is required.
  • A passion for supporting individuals with unique needs is essential.
  • Valid Driver's License, reliable transportation, and acceptable driving record.
  • Must pass company background and reference checks.
