Key Responsibilities:
As a Care Specialist, you will be responsible for providing assessment and care management services to clients in collaboration with other professionals. You will also participate in our duty service and contribute to the development of the service.
Requirements:
- A professional social work qualification
- A current driving license and use of a vehicle (with reasonable adjustments considered for candidates with disabilities)
Benefits:
