About the Role

We are seeking a highly skilled and experienced Geriatric Care Specialist to join our team in North Powys. As a key member of our Frailty team, you will play a vital role in coordinating the provision of care for our most vulnerable patients.

Key Responsibilities
  • Provide expert advice and specialist nursing knowledge to individuals with severe frailty, their families, and carers.
  • Work closely with families in clinic and community settings to develop personalized care plans.
  • Use your clinical expertise to make informed decisions and provide direction for the Primary Care and Community Frailty team.
Requirements
  • Registered Nurse with current NMC registration.
  • Relevant master's degree or equivalent level of knowledge, skills, and experience.
  • Evidence of continuing professional development at degree level.
  • Working knowledge of legislation relevant to the field.
Salary and Benefits

The salary for this position is £45,000 - £55,000 per annum, depending on experience. We offer a range of benefits, including opportunities for career development and a healthy work-life balance.
