Job Overview

We are seeking a skilled Community Mental Health Nurse to join our multidisciplinary team in Swansea. This role will involve working as a Care Coordinator, delivering high-quality community mental health services under the framework of the Mental Health Measure (2010) Wales.

Main Responsibilities

  • Assess, implement, and evaluate care and treatment plans for individuals on the caseload in collaboration with service users and carers.
  • Work autonomously under indirect supervision and in conjunction with other health colleagues to provide a supportive service for those under EIP care.

Person Specification

  • Registered Mental Health Nurse
  • Evidence of post-registration study, e.g., postgraduate diploma or equivalent registration experience.
  • Ability to demonstrate clinical knowledge, current legislation, and care and treatment for adults with mental health needs.
  • Experience of multidisciplinary and multi-agency working, risk assessment, and risk management.

Employer Certification

This post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and will require a submission for Disclosure to the Disclosure and Barring Service.
