About the Role

We are seeking a highly skilled and dedicated Mental Health Nurse to join our team at Swansea Bay University Health Board. As a key member of our community mental health team, you will play a vital role in providing high-quality care to patients with complex mental health needs.

Key Responsibilities
  • Conduct comprehensive assessments of patients to identify their mental health needs and develop personalized care plans.
  • Work collaboratively with multidisciplinary teams to provide evidence-based interventions and support patients in achieving their recovery goals.
  • Provide expert nursing care to patients in their homes, community settings, and other locations as required.
  • Develop and maintain strong relationships with patients, families, and carers to ensure effective communication and support.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote best practice in mental health care.
Requirements
  • Registered Mental Health Nurse with a strong background in community mental health care.
  • Experience of working with older adults with mental health needs.
  • Knowledge of Mental Health Measure (2010) and other relevant legislation.
  • Ability to work autonomously and as part of a team, with excellent communication and interpersonal skills.
  • Willingness to travel to patients in a timely manner and work in a variety of settings.
