Job Overview

An exciting opportunity has arisen to join a dynamic and passionate Community Mental Health Team (CMHT) within Swansea Bay University Health Board as a Care Coordinator. The role will involve working as part of a multidisciplinary team to deliver a high-quality community mental health service under the framework of the Mental Health Measure Wales, while maintaining the Recovery ethos at the forefront of nursing care.

Main Duties of the Job

To assess, implement, and evaluate agreed care and treatment plans for each individual on the caseload, which has previously been agreed with the service user/carers. To work autonomously but under the indirect supervision of a Band 7 registered nurse/team manager and in conjunction with other health colleagues in the delivery of a supportive service for those people under the care of Early Intervention in Psychosis (EIP).

Detailed Job Description and Main Responsibilities

The successful candidate will be expected to work autonomously but also as part of a team, which will involve mental health assessments and providing a point of contact for existing service users in the absence of their care coordinator. The role will also occasionally involve facilitating key clinics within the CMHT, undertaking a range of physical health checks or medication administration in line with local policy.

Qualifications

Essential Criteria

  • Registered Mental Health Nurse
  • Evidence of post-registration study, e.g., postgraduate diploma or equivalent registration experience

Knowledge

Essential Criteria

  • Ability to demonstrate clinical knowledge base and implications of current legislation, i.e., Mental Health Act 1983, Mental Health (Wales) Measure 2010, Mental Capacity Act, Deprivation of Liberty
  • Able to demonstrate a good knowledge of the current care and treatment for adults with mental health needs
  • An understanding of the Clinical Governance agenda

Experience

Essential Criteria

  • Experience of multi-disciplinary and multi-agency working
  • Experience of risk assessment and risk management
  • Experience at Band 5 or above in a related clinical environment

Desirable Criteria

  • Experience of leadership within a team
