Community Mental Health Nurse Specialist

We are seeking a highly skilled and experienced Registered Mental Health Nurse to join our team in Swansea Bay University Health Board. As a Community Mental Health Nurse, you will be responsible for providing high-quality care to patients with mental health needs in the community setting.
Key Responsibilities- Conduct comprehensive assessments of patients' mental health needs and develop individualized care plans.
- Provide evidence-based interventions and treatments to patients, including medication management and psychotherapy.
- Work collaboratively with multidisciplinary teams, including social workers, psychologists, and psychiatrists, to ensure comprehensive care.
- Provide education and support to patients and their families to promote mental health and well-being.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
- Qualification as a Registered Mental Health Nurse.
- Experience of working with older adult mental health patients.
- Knowledge of Mental Health Measure (2010).
- Ability to travel to patients in a timely manner.
