About the Role

We are seeking a highly motivated and compassionate individual to join our team as a Community Support Worker at Calderdale and Huddersfield NHS Foundation Trust. As a key member of our multi-disciplinary team, you will play a vital role in providing support to adults of all ages who require assistance to maintain their independence within their home setting.

Job Summary

As a Community Support Worker, you will work closely with individuals, their families, and carers to promote and maintain their independence, dignity, and choice. You will undertake medical, nursing, or therapy-related treatment programs or social care interventions to support and enable people to live safely and independently in their own homes.

Main Responsibilities
  • Implement treatment plans and protocols, and make minor adjustments within predetermined limits
  • Monitor progress and feed back to registered practitioners or assistant practitioners
  • Assist registered practitioners with complex treatment
  • Assess for, order, and issue basic equipment to protocols and established guidelines
  • Participate in new initiatives and future changes in care delivery, including the Virtual Ward
  • Support hospital discharges and prevent unnecessary admissions to hospitals
  • Work across the whole of Calderdale, caring and supporting individuals within their own homes

Person Specification
  • Essential Qualifications and Experience:
    • GCSE Level English and Maths or Equivalent
    • NVQ L3 or equivalent experience
    • Willing and able to participate in Competency Work Base Training Programme
    • Willingness to undertake additional and mandatory training as required
  • Desirable Qualifications and Experience:
    • Previous based competency training in a relevant field
    • Recent relevant experience working and communicating with the public in social care/health care
    • Recent relevant experience of dealing with adult clients with complex needs and their carers
    • Recent experience of working in an intermediate care service
    • IT literate (email/internet)
    • Understanding of the needs and issues concerning illness and disability
    • Understanding of rehabilitation and re-ablement principles
    • Understanding of enabling a person to learn or re-learn skills
    • Understanding of the issues when working in the user's own home
  • Communication and Relationships:
    • Recent relevant experience of instructing adults to perform practical tasks
