Job Summary

We are seeking a skilled and compassionate Community Care Worker to join our team. As a Community Care Worker, you will play a vital role in providing support to adults of all ages who require care and support to maintain their independence in their own homes.

Main Responsibilities
  • Implement treatment plans and protocols, based on competencies and guidelines.
  • Monitor progress and provide feedback to registered practitioners or assistant practitioners.
  • Make minor adjustments to treatment programs within predetermined limits.
  • Assist registered practitioners with complex treatment and interventions.
  • Assess for, order, and issue basic equipment to patients, following established guidelines.
Key Skills and Qualifications
  • GCSE Level English and Maths or equivalent.
  • NVQ L3 or equivalent experience.
  • Ability to work within competencies and local protocols.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
