About the Role

We are seeking a highly skilled and compassionate Senior Rehabilitation Assistant to join our team at Calderdale and Huddersfield NHS Foundation Trust. As a key member of our multi-disciplinary team, you will play a vital role in providing support to adults of all ages who require rehabilitation and re-ablement services in their own homes.

Key Responsibilities
  • Implement treatment plans and protocols to support individuals in maintaining their independence and dignity.
  • Monitor and review individuals' progress, providing feedback to registered practitioners and making adjustments to treatment programs as needed.
  • Assist registered practitioners with complex treatment and provide support with daily living activities.
  • Contribute to the discharge of individuals into the care of another service, ensuring a smooth transition and continuity of care.
  • Provide advice and information to individuals on managing their own condition, promoting self-care and self-management.
  • Deliver exercise sessions and mobility programs to improve health and wellbeing.
  • Undertake personal hygiene for individuals unable to care for themselves.
  • Collect, transport, and maintain healthcare equipment and medical devices, ensuring they are in safe working order.
Requirements
  • GCSE Level English and Maths or equivalent.
  • NVQ L3 or equivalent experience.
  • Willingness to participate in competency work-based training and undertake additional training as required.
  • Recent relevant experience working with adults with complex needs and their carers.
  • IT literacy and understanding of rehabilitation and re-ablement principles.
  • Ability to work in a community setting and as part of a multidisciplinary team.
