Vulnerable Adult Support Specialist

We are seeking a dedicated and compassionate Vulnerable Adult Support Specialist to join our team in Lewisham. In this role, you will provide expert support to vulnerable adults, empowering them to make informed decisions about their resources and services.
You will work closely with residents, their unpaid carers, and multi-disciplinary teams to deliver high-quality community-based packages that prioritize independence and personalization. Your strength-based approach will facilitate individual and community capacity, social inclusion, and community networks, leading to improved outcomes for those we serve.
A key aspect of this role is building strong relationships and working collaboratively with stakeholders to ensure seamless service delivery. You will be responsible for promoting independence, personalization, home first, and discharge assessments, ensuring that residents receive the best possible support to sustain or improve their quality of life.
To succeed in this position, you will need:
- A qualified Social Worker (QSW) status, including qualification and registration with Social Work England.
- A minimum of 2 years' experience in social work.
- The right to work in the UK.
