Discharge Coordination Specialist

The Community Care Navigator will play a critical role in ensuring that patients receive seamless care throughout their treatment journey. This position requires strong communication skills, attention to detail, and the ability to work effectively with multidisciplinary teams.

Key Responsibilities- Develop and maintain relationships with healthcare professionals and community services to ensure effective discharge planning
- Work collaboratively with multidisciplinary teams to ensure seamless transitions of care
- Provide senior support to the team to improve the quality of service for patients
- Possession of teaching/mentorship qualification
- Experience of working within a busy day-to-day NHS service
- Prior relevant experience of working with cancer services
This full-time position comes with a salary range of £44,806 to £53,134 per annum, inclusive of HCAS pro rata. We also offer a range of benefits, including opportunities for professional development and training.
