Senior Clinical Nurse Specialist

As a Senior Clinical Nurse Specialist at The Ashton Care Home, you will be responsible for delivering high-quality, patient-centered care to our residents. You will work closely with multidisciplinary teams to develop and implement personalized care plans that prioritize the needs and preferences of each individual.
Key Responsibilities:
- Collaborate with residents, families, and healthcare professionals to design and implement care plans
- Ensure accurate and thorough documentation of resident interactions and conditions
- Lead and guide nursing teams to deliver top-notch care
- Oversee shift handovers to ensure seamless transitions and continuity of care
- Monitor resident health and safety, managing specific needs and administering medications as required
- Foster strong relationships with residents, families, and healthcare professionals
- Support residents in daily activities, promoting independence and autonomy
- Act on feedback to ensure resident health and well-being are prioritized
- Uphold the dignity and individuality of every resident
- Facilitate social interactions and activities to enrich the lives of residents
- Maintain a vigilant eye on health and safety, ensuring a secure environment for all
- Advocate for balanced nutrition and hydration, monitoring residents at risk of weight changes or dehydration
About You:
- You are a Registered General or Mental Health Nurse (RGN or RMN) with an active NMC Nursing PIN
- You have a passion for elderly care and a commitment to improving the lives of our residents
- You possess excellent communication and interpersonal skills, with a natural ability to connect with people
