Clinical Nurse Coordinator

We are looking for a Clinical Nurse Coordinator to join our team at ComForCare. As a Registered Nurse (RN), you will be responsible for performing the initial consumer assessment and reassessment, as needed, with input from the consumer. You will educate, coach, and monitor consumers and caregivers on disease process, medications, treatments, and medical/nursing plan of care, ensuring the consumer's needs are met.
You will prepare clinical notes and submit paperwork in a timely manner per Agency policy. Additionally, you will delegate tasks to caregivers and licensed staff according to established standards, skill level required, and ability to perform the task to achieve the desired outcome.
The successful candidate will perform supervisory visits and monitor outcomes of delegation to ensure all direct care staff are adhering to the plan of care to meet the needs of the consumer. A registered nurse degree and Current CPR certification are required. With a minimum of one (1) year of acute care nursing experience as a RN, you will be able to provide care in compliance with Agency protocols in a competent, safe, and timely manner.
