We are seeking a skilled and compassionate Nurse Care Coordinator to join our team at Cedar Care Homes.

Job Title: Nurse Care Coordinator
Key Responsibilities- Develop and implement individualized care plans for residents, working closely with the multidisciplinary team to ensure their needs are met.
- Lead by example, demonstrating excellent attitude and care practices to promote a positive work environment.
- Manage the care of allocated residents, ensuring their dignity and well-being are maintained at all times.
- Supervise and support the performance of named carers and care assistants, promoting a culture of excellence and continuous improvement.
- Registered Nurse qualification (RN1 or RN3) with post-registration experience in elderly care and dementia care.
- Strong leadership and communication skills, with the ability to work effectively in a team.
- Ability to adapt to changing situations and priorities, while maintaining a calm and composed demeanor.
- Proven track record of delivering high-quality patient care, with a focus on resident-centered care.
- Excellent time management and organizational skills, with the ability to prioritize tasks and manage multiple responsibilities.
- A competitive salary of £38,000 - £45,000 per annum, depending on experience.
