About the Job: We are seeking an experienced Adult Care Coordinator to join our team at HC-One. As an Adult Care Coordinator, you will be responsible for delivering exceptional care to our residents, promoting their independence and well-being.

Key Responsibilities:

  • Develop and implement care plans that cater to the unique needs of each resident
  • Collaborate with Registered Nurses and Care Managers to ensure seamless care delivery
  • Provide guidance and support to Care Assistants to maintain high standards of care
  • Maintain accurate records and report any concerns or incidents

Requirements:

  • A Level 2 Diploma S/NVQ in Adult Care or equivalent
  • Experience working in adult care, preferably in a care home setting
  • Knowledge of caring for people with dementia and ability to develop person-centered care plans
