Support Worker for Adults with Complex Needs

We are seeking a Support Worker to join our Adults with Complex Needs team in the Chester and Ellesmere Port area. This role involves providing tailored support to adults with learning disabilities, promoting their independence and well-being.
The successful candidate will have prior experience working with adults with complex needs and a sound understanding of relevant legislation, policies, and best practices in adult social care.
Key Responsibilities:- Conduct comprehensive assessments to identify needs and strengths of service users.
- Develop and implement person-centered care plans that promote independence and enhance quality of life.
- Support the team's duty rota, responding promptly to urgent cases and ensuring continuity of care.
- Apply relevant legislation, including the Care Act and Mental Capacity Act, to uphold the rights and safety of service users.
- Maintain accurate records in line with council policies and procedures.
The estimated annual salary for this position is £41,600 - £58,200 based on a 40-hour workweek. The hourly rate is £25-£35.
