Job Summary:
We are seeking an Advanced Pharmacist Practitioner to join our team. As an independent prescriber, you will work autonomously, making clinical decisions, including in situations with risk, uncertainty, and limited evidence. You will be responsible for a caseload of patients and collaborate with healthcare professionals to ensure their ongoing care.
Key Responsibilities:
- Act as an independent prescriber for scheduled and unscheduled care, liaising with healthcare professionals to ensure the ongoing care of patients.
- Work autonomously, making clinical decisions, including in situations with risk, uncertainty, and limited evidence.
- Be responsible for a caseload of patients and collaborate with healthcare professionals to ensure their ongoing care.
- Be innovative and forward-thinking, supporting the development of the pharmacy service within the GP practice.
Requirements:
- Good communication, organisational, and IT skills.
- Ability to work with patients and other healthcare professionals.
- Substantial experience as a qualified pharmacist.
- Registration as a pharmacist with the General Pharmaceutical Council.
- Independent prescriber status.
- Postgraduate qualification in clinical pharmacy or equivalent competence.
Working Arrangements:
Three positions available, working 37 hours per week. A UK driving licence and car ownership are essential for this role.
