Driving Requirement: A valid UK Manual Driving License is essential for operating company vehicles.

As a Care Support Assistant, you will be a vital support to individuals who may face challenges due to age, health issues, or other vulnerabilities. Key Responsibilities:
Deliver practical and emotional assistance (including phone check-ins, welfare assessments, shopping support, etc.) to clients.Provide responsive, tailored support to adults post-hospitalization to facilitate early discharge.Act on referrals from healthcare professionals and community clinicians.Evaluate client needs and develop personalized support plans.Assist clients during their recovery phase after hospital discharge or to avert readmission.Transport clients to medical appointments or assist with prescription pickups and grocery shopping.
Qualifications for Success:
Familiarity with NHS and social care services.A commitment to enhancing service quality for client benefit.Professionalism in handling inquiries with discretion and confidentiality.Flexibility in working hours to accommodate varying needs.This role does not involve personal care or medication administration.
