Job Description

Role Summary

We are seeking a skilled and experienced Social Worker to join our Community Mental Health Team in Dunstable & Leighton Buzzard area.

As a member of the multi-disciplinary team, you will provide high-quality community mental health interventions, including care act assessments & reviews, safeguarding, hospital discharge, and MDT working.

Key Responsibilities

  • To provide comprehensive assessments and interventions for individuals with mental health needs.
  • To work collaboratively with the multi-disciplinary team to develop and implement care plans.
  • To provide support and guidance to individuals, families, and carers.
  • To maintain accurate and up-to-date records of client interactions.

Requirements

We are looking for a qualified and experienced Social Worker who is passionate about delivering high-quality community mental health services. You will have excellent communication and interpersonal skills, and be able to work effectively in a fast-paced environment.
