Job Overview

We are seeking an experienced Care Practitioner to join our team as a Mental Health Support Specialist. This role will provide you with the opportunity to work with individuals who require support and guidance in their daily lives.

Key Responsibilities

  • Deliver holistic assessments and develop personalized plans for individuals supported by Gray Healthcare.
  • Collaborate with multidisciplinary teams to ensure seamless delivery of care services.
  • Work closely with individuals to identify and address their needs, promoting independence and empowerment.
  • Provide coaching and guidance to support staff on best practices and evidence-based interventions.

Requirements and Qualifications

To be successful in this role, you will have:

  • A minimum of 6 months of experience in a relevant field, such as mental health, learning disability, or complex needs.
  • An understanding of trauma-informed care principles.
  • Excellent communication and interpersonal skills, including written and verbal communication.
  • Ability to work flexibly, including overnight stays when required.
