Job Title: Advanced Nurse Practitioner


Job Summary:


We are seeking an experienced Advanced Nurse Practitioner to join our team at HCRG Care Group. As an Advanced Nurse Practitioner, you will play a crucial role in delivering high-quality, patient-centred care to our service users. Your expertise in complex care and ability to work autonomously will be invaluable in ensuring the best possible outcomes for our patients.


About the Role:



  • Deliver high-quality, patient-centred care to our service users
  • Work autonomously and as part of a multidisciplinary team
  • Develop and implement care plans that meet the individual needs of our patients
  • Collaborate with other healthcare professionals to ensure seamless care provision
  • Contribute to the development of clinical policies and procedures

Key Responsibilities:



  • Assess and diagnose patients' conditions, develop and implement care plans, and evaluate patient outcomes
  • Prescribe medication and treatment plans, as appropriate
  • Develop and maintain effective relationships with patients, families, and carers
  • Participate in multidisciplinary team meetings and contribute to the development of clinical policies and procedures
  • Stay up-to-date with the latest clinical guidelines and best practices

Requirements:



  • Registered Nurse with a current registration
  • Relevant postgraduate qualifications, such as an MSc in Advanced Clinical Practice
  • Experience of working in an acute care setting
  • Ability to work autonomously and as part of a multidisciplinary team
  • Excellent communication and interpersonal skills
