About the Role
Connect Executive Search Group is seeking an experienced Diabetes Specialist Nurse Advisor to join our nationwide team. As a key member of our primary care team, you will play a crucial role in improving patient outcomes for individuals with Type 2 diabetes.
Job Description
This is an exciting opportunity to work as part of a dynamic and forward-thinking team. The successful candidate will be responsible for delivering high-quality care to patients with Type 2 diabetes, in line with local and national guidelines (NG28). You will work proactively to identify sub-optimally controlled patients, optimize their oral therapy, conduct point-of-care testing, and refer for specialist insulin treatment when necessary.
Required Skills and Qualifications
To be considered for this role, you will need:
- A minimum of 3 years' experience managing diabetes within primary care
- A Diabetes diploma or equivalent qualification
- Excellent knowledge and skills in the optimization of oral hypoglycaemics
- Fully qualified and registered with the NMC
- Full driving license and IT literacy, including proficiency in primary care systems
