We are seeking an experienced Type 2 Diabetes Care Manager to join our team in primary care. As a key member of our nationwide team, you will play a crucial role in delivering high-quality patient care and improving outcomes.

About the Role

This is a full-time or part-time opportunity (4 days per week minimum) with a competitive annual salary of approximately £45,000, plus a bonus incentive scheme worth up to 10% of your annual salary. You will be responsible for identifying sub-optimally controlled patients with Type 2 diabetes, optimising their oral therapy, conducting point of care testing, and referring for specialist insulin treatment where necessary.

Key Responsibilities

  • Deliver high-quality patient care, in line with local and national Diabetes guidelines (NG28)">
  • Identify and engage with patients who require optimisation of their oral hypoglycaemics">
  • Conduct point of care testing and refer patients for specialist insulin treatment when necessary">
  • Exercise corporate nursing responsibilities">

Requirements

To be successful in this role, you will need:

  • ">
  • A minimum of 3 years experience managing diabetes within primary care">
  • Degree-level qualification in a relevant field, such as Diabetes Diploma">
  • Full driving licence and current NMC registration">
  • IT literacy, including primary care systems">
