Job Summary

We are seeking a highly motivated and experienced Primary Care Nurse Clinician to join our team as a Diabetes Specialist Nurse Advisor. This is an exciting opportunity to work as part of a dynamic and forward-thinking team, delivering high-quality care to patients with Type 2 diabetes.

Duties and Responsibilities

The successful candidate will be responsible for:

  • Delivering high-quality care to patients with Type 2 diabetes, in line with local and national guidelines (NG28)
  • Working proactively to identify sub-optimally controlled patients and optimizing their oral therapy
  • Conducting point-of-care testing and referring for specialist insulin treatment when necessary
  • Exercising corporate nursing responsibilities, including education, support, and advice to people with Type 2 diabetes

Requirements

To succeed in this role, you will need:

  • A minimum of 3 years' experience managing diabetes within primary care
  • A Diabetes diploma or equivalent qualification
  • Excellent knowledge and skills in the optimization of oral hypoglycaemics
  • Fully qualified and registered with the NMC
  • Full driving license and IT literacy, including proficiency in primary care systems

Estimated Salary
