Primary Care Nurse Consultant

We are seeking an experienced Primary Care Nurse Consultant to join our team in primary care across the country. The successful candidate will be responsible for developing and implementing effective diabetes management plans, conducting regular reviews and assessments of patients with Type 2 diabetes, and collaborating with healthcare professionals to ensure comprehensive care.
Key Responsibilities
- Develop and implement individualised care plans for patients with Type 2 diabetes
- Conduct regular reviews and assessments of patients with Type 2 diabetes
- Collaborate with healthcare professionals to ensure seamless care transitions
Requirements
- A minimum of 3 years' experience in diabetes management
- Strong knowledge of local and national guidelines
- Excellent clinical and communication skills
Our Benefits Package
- Annual salary: £45,000 plus bonus (up to 10% of annual salary)
