Clinical Diabetes Care Specialist

National Services for Health Improvement is committed to delivering high-quality care and improving patient outcomes in diabetes management. We are seeking a highly skilled Clinical Diabetes Care Specialist to join our nationwide team of full-time professionals working in primary care.
About the RoleYou will play a crucial part in optimising oral therapy, conducting point-of-care testing, and referring patients for specialist insulin treatment when necessary. Your commitment to delivering exceptional care will contribute to improved health outcomes for individuals with Type 2 diabetes.
Key Responsibilities- Provide education, support, and advice to people with Type 2 Diabetes
- Optimise oral hypoglycaemics through knowledge and expertise
- Identify sub-optimally controlled patients and implement targeted interventions
- Diabetes diploma or equivalent qualification
- A minimum of 3 years experience managing diabetes in primary care
- Excellent communication and interpersonal skills
- Full driving licence
- Current NMC registration
- IT literacy in primary care systems
