Chronic Disease Management Nurse

Are you a motivated and compassionate nurse looking to take your career to the next level? We have an excellent opportunity for a Long Term Conditions Nurse to join our Primary Care team at HMP Foston Hall, a closed category women's Prison and Young Offenders Institution located in Derbyshire, England.
This is a unique chance to develop your skills and experience in a challenging yet rewarding environment, where you will work closely with other healthcare services and departments within the prison to provide high-quality care to patients.
The role requires you to identify immediate health needs, including any external services or prescribing requirements that prisoners may have received prior to remand or repatriation onto a wing. You will also be responsible for ensuring comprehensive handovers of services when patients transfer across wings or custodial care settings.
In exchange for your hard work and commitment, you will receive a competitive annual salary of up to £47,466 per annum (FTE), depending on your experience, plus a range of benefits that support your career development and overall well-being.
To be eligible for this position, you must have relevant NMC registration, experience of managing long-term conditions, and knowledge of evidence-based practice and clinical governance. While experience in a prison setting is not necessary, you must be able to adapt to working in a secure environment.
