Nurse Specialist in Chronic Care

This is an exciting opportunity for a skilled Long Term Conditions Nurse to join our dynamic Primary Care team at HMP Foston Hall, a closed category women's Prison and Young Offenders Institution located in Derbyshire, England.
We are looking for a dedicated professional with expertise in managing long-term conditions to provide high-quality care to our patients. The successful candidate will have the opportunity to develop their skills and experience in a challenging yet rewarding environment.
The role involves working closely with other healthcare services and departments within the prison, such as Pharmacy, Education, Offender Management, Substance Misuse, and Physical Education, to ensure effective collaboration and stability for patients receiving complex care.
In return for your hard work and dedication, you will receive a competitive annual salary of up to £47,466 per annum (FTE), depending on your experience, along with a range of benefits that support your career development and well-being.
To be considered for this role, you will need to have relevant NMC registration, experience of managing long-term conditions, and knowledge of evidence-based practice and clinical governance. Experience in a prison setting is not essential, but you must have the ability to work effectively in a secure environment.
