NHS Community Care Coordinator

The Lancashire & South Cumbria NHS Foundation Trust is seeking a highly skilled Clinical Lead Nurse to coordinate community care services. As a Clinical Lead Nurse, you will work closely with an established multi-disciplinary team to develop and deliver specialist programmes of care and risk management plans.
You will be responsible for ensuring high-quality patient care, acting as a role model, and demonstrating expertise in clinical supervision. Key responsibilities include managing clinical caseloads, mentoring and supervising junior members of staff, and contributing to the evaluation of risk management plans.
What We Offer:
- A competitive salary of £60,000 - £80,000 per annum, depending on experience

Person Specification:
- Registered Mental Health Nurse with NMC
- Post-registration experience and/or evidence of formal courses and training at post-graduate diploma level
- Mentorship Course, including Clinical Supervision Training
- Non-Medical Prescribing Qualification (or willing to work towards this)
