About the Role

The Pendle Medical Partnership is seeking a highly skilled and compassionate Care Navigator to join our team. As a Care Navigator, you will play a vital role in supporting our patients and clinicians to deliver the best possible care.

Key Responsibilities
  • Process appointment requests and administrative queries via our electronic consultation system, ensuring patients are directed to the most appropriate healthcare professional.
  • Deal with patients by telephone and in person, utilizing our clinical system to provide efficient and effective care.
  • Work in accordance with our practice protocols, policies, and procedures to ensure seamless delivery of care.
  • Handle general enquiries, make follow-up appointments, and respond to tasks and queries from the practice team.
  • Process prescriptions, register new patients, and update patient information on our clinical system.
  • Provide additional cover for annual leave and sickness absence, ensuring continuity of care for our patients.
Requirements
  • Experience working in a busy office environment, dealing with confidential information, and interacting with patients in a healthcare setting.
  • Knowledge of the EMIS system and experience working in a team setting are desirable.
  • Good standard of general education, ability to multitask, and remain calm under pressure are essential.
  • Computer literacy, including Office 365 packages, email, and the internet, is required.
